McClerklin Skin & Laser Center works to serve our patients with the utmost care to their medical needs. In order to streamline the process of your visit, we have listed the paperwork that you will need for your initial visits and follow-up visits.
Please print and fill out these forms and bring them with you to your appointment. DO NOT mail or fax the forms, as they will NOT be considered.
New Patient Forms
If you are a new patient or have not been seen by the doctor in awhile, you will need to fill out the patient forms below. Once you arrive at the office, you will be seen after your insurance has been verified and your co-pay received. To decrease your wait time, we recommend that you print, fill-out, and bring the following four documents with you to your initial visit:
Established Patient Forms
All established patients must update their information once every six months. Please print out the Patient Registration Form below and bring it with you to your upcoming appointment.
Skin Care Protocols
If you have had a procedure done at McClerklin Skin & Laser Center, you must follow the appropriate pre and post-treatment protocols to maximize your results.
